Registered Nurse (RN) | Case Manager Location: Oklahoma City, OK Agency: Cynet Health Pay: $2,512 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Job Title: Nursing Case Manager Profession: Manager Specialty: Case Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: Three (3) years clinical nursing experience in an acute care facility License: Current RN License Certifications: - Current Basic Life Support issued by the American Heart Association required upon hire Must-Have: - Knowledge of nursing practices and procedures - Strong clinical assessment skills - Knowledge of resources available in the community for patient recovery - Understanding of third party payer issues concerning reimbursement and regulations - Ability to explain complex information in an understandable manner - Strong interpersonal skills for gathering necessary patient information Description: As a member of a multidisciplinary care management team, the nursing case manager is responsible for coordinating the care and services of selected patient populations. The case manager comprehensively assesses patient goals, including biophysical, psychosocial, environmental, discharge planning needs, and financial status. They closely monitor patients' progress for changes in medical needs affecting treatment. The case manager procures services and advocates on behalf of patients and families with limited resources. They work closely with third party payers to resolve issues impacting patient care continuity. Coordination and direction of discharge planning involve engaging with family members, healthcare teams, and the community. The case manager reviews admission data to determine the suitability of the level of care according to established criteria.
