Job Description Certification Details - BLS Job Details -
Behavioral Health Unit with med/surg level medical needs. - 1:6
patient ratio, 12-bed unit, 2 nurses/2 CNA assistive staff. -
Cerner EHR, ceil blue scrubs. Job Requirements - Must have current
nursing license with Nursys verification within 30 days prior to
start. - Completion of FACIS Questionnaire if any disciplinary
actions. - Completion of Ascension Cerner Test-Out. - Completion of
Ascension Color Vision test prior to start and updated every 3
years. - Completion of Ascension Core Competencies within a year of
start, updated yearly with minimum 80% score. - Completion of
Ascension EKG Exam prior to start with passing score of at least
80%. - Completion of Ascension Onboarding Requirements Education
Attestation. - Completion of Ascension Nursing Contract Worker
Onboarding onsite. - Completion of Ascension Nova StatStrip prior
to clearance. - Completion of CPI Blue Card Nonviolent Crisis
Intervention with Advanced Physical Skills or MOAB De-Escalation
Course prior to start. - Documentation of required vaccinations and
health screenings including MMR, Hepatitis B, Influenza Vaccine,
Varicella, Tdap, Positive PPD or IGRA, and Health Statement. - Fit
Test required annually. - Driver's License/State ID must be valid
and match address requirements. - Completion of Ascension 12 Panel
Drug Screen within 30 days prior to start and updated annually. -
Verification of education relevant to clinical work within 90 days
of start. - Provision of professional references and employment
verifications as required. Schedule Information - Contract length
is 13 weeks. Unit Specific Information - Behavioral Health Unit
with med/surg level medical needs. - 1:6 patient ratio, 12-bed
unit, 2 nurses/2 CNA assistive staff. Additional Details - Cerner
test results and Training Checklist with signed attestation
required. - Forward facing headshot with a white or blue background
required. - Ascension Unit Specific Checklist optional for
onboarding but needs to be uploaded by unit leader within a week. -
Flu season is October 1, 2025 through March 31, 2026; exemptions
accepted but mask required. - Post-assignment evaluation optional.
- Legal documentation of name change optional.
