insurance As the Patient Care Orders Specialist, you’ll contribute
to our success in the following ways: - Establishes and maintains
effective and efficient systems for orders management to ensure
accurate admission processes are followed. - Obtains signed orders
for skilled care remotely meeting weekly productivity expectations
to enable Check-In program achieve admission targets and other
goals. - Collaborates with Check-In team, physician, and Sales team
to obtain all orders and clinical documentation to then process the
referral to the branch for review and communicate the need to
obtain any further documentation needed for admission. - Verifies
accuracy of clinical documentation received as part of orders
management, including home health orders, face-to-face
documentation, clinical notes from prior encounters, and recent
history and physical documentation. - Denotes incomplete
information or documentation prior to sending to appropriate branch
level admission coordinator (e.g., pre-admission coordinator,
clinical team manager). - Documents or tracks all orders and
clinical documentation received by the agency. - Answers all
inquiries, regarding agency services, and disseminates appropriate
information including referrals to community resources. - Maintains
current knowledge of, and effects of compliance with all federal,
state, and local regulations. - Maintains organized, effective, and
efficient systems and communication to ensure the continuity of
quality patient care and timely initiation of care. - Escalates
delays in orders and pending referral follow-up with the
appropriate team to ensure timely response to patient care needs. -
Follows regulatory requirements and timelines when obtaining signed
orders for care. - Assists in evaluating agency operations and
systems and identifies ways to enhance workflow and productivity. -
Transfers referrals or healthcare needs to appropriate internal
partner or supervisor for review. - Participates in clinical record
audits, utilization reviews and Peer Review and Quality Improvement
programs. What is Required? - Associates Degree in Nursing or
associated Diploma/Certificate in Nursing is required. - 2 years’
experience as an LPN/LVN - Current Certified LPN/LVN - Proficiency
with MS Office Suite (Word, Excel, Outlook) - Prior experience with
enterprise applications (e.g., Homecare Homebase, Workday, etc.) -
Excellent verbal and written communication skills. - Experience
with Medicare/Medicaid home health care benefits, policies and
procedures is preferred. You will report to the Clinical Program
