team members. Administer medications, wound care, and other
personalized interventions as outlined in patient care plan.
Educate patients and their families on health maintenance and
disease prevention. Document patient care services and outcomes for
clinical records. Collaborate with healthcare team members to
ensure optimal patient care. Provide emotional support and
compassionate care to patients and families. Strong clinical skills
in a home health setting Excellent communication and interpersonal
skills Ability to work independently with minimal supervision
Proficiency in electronic healthcare documentation Critical
thinking and problem-solving skills Medical Insurance Dental
