Job Description:

AccentCare is seeking a dedicated Registered Nurse for our Home Health division to provide direct nursing care to our clients in their homes. This position is crucial for ensuring the delivery of high-quality health care services, enabling patients to recover or manage their conditions comfortably and safely at home. As part of our team, you will be instrumental in crafting personalized care plans and ensuring the emotional and physical well-being of our patients, supported by a holistic approach toward health and recovery. Assess the patient’s condition during every visit and chart your observations Perform evaluation tasks, including vital signs and medication review Administer medication as prescribed by the patient’s physician Educate patients and their families on proper home health care strategies and procedures Coordinate with physicians and other healthcare professionals for creating and evaluating customized care plans Maintain accurate health care records and reports Provide palliative care as needed to help alleviate the patient’s symptoms and pain Proficient in providing general nursing care Capable of patient health assessment Strong organizational and communication skills Ability to work independently in a home-setting environment Proficient in using healthcare software and systems Medical Insurance Dental Insurance Vision Insurance 401k Active Registered Nurse license CPR certification Valid driver’s license and reliable transportation
