Responsibilities
The Wound Care Nurse assesses patient care needs and facilitates
interventions for patient care, treatment and education related to
enterostomal therapy/skin care issues. Serves as a consultant to
nursing, medical and interdisciplinary health team members in the
area of enterostomal therapy.
- Provide consultative and direct clinical services to persons
with fecal and urinary diversions, wound management problems,
pressure and vascular ulcers, cutaneous fistulae, problems with
cutaneous tubes and drains and skin care needs associated with
incontinence and any other wound care case as presented in the
department.
- Select/mark stoma site, observe/report stoma condition.
- Coordinate care of patient's complex draining wounds, fistulae,
drains and tubes.
- Intervene clinically for patients with various states of skin
breakdown, focusing on Stage III and IV wounds directly. Provides
consultation on other wounds to facilitate appropriate care and
intervention.
- Report changes/updates to physician and healthcare team.
- Make recommendations for a plan of care.
- Prepares and maintains appropriate teaching materials for
patients/families and staff regarding skin/ostomy wound care.
- Develop, implement and revise policies, procedures,
competencies and plans of care related to care of patients with
ostomies, draining wounds/pressure ulcers and skin care of patients
in general.
- Coordinates for the placement of wound care equipment to
facilitate avoidance of skin breakdown or the appropriate treatment
for those with existing/developing wounds.
- Contributes to a reduction in the development of wounds during
inpatient treatment.
- Coordinates prevalence and incidence studies.
- Demonstrates competence in assembling and operating equipment
for patient care.
- Documents assessments, plan of care implemented interventions
and patient's response to intervention/treatment according to
policy utilizing appropriate forms/screens.
- Coordinates for authorization of wound care equipment for
inpatients and patients being discharged.
- Demonstrates awareness of legal issues in all aspects of
patient care and strives to manage situations in a reduced risk
manner.
- Perform other duties as required and assigned.
Qualifications
- Current RN licensure from the State of Oklahoma required.
- ADN required, BSN preferred.
- WOCN certification required within 18months of being
hired.
- Enterostomal Therapy/Wound Care experience preferred.
- Current BLS required.
- Experience preferred.
- Must successfully complete the pre-employment assessment.
