Job Description:

Responsibilities The Licensed Practical Nurse (LPN) practices under the direction or supervision of the professional registered nurse. The LPN implements nursing care for assigned patients and assists the RN in the assessment, planning, intervention, and evaluation of nursing care. This includes demonstrating competency in strong basic patient care skills, clerical/courier duties, stocking, transporting, specific patient care duties, and other departmental duties as requested. May be floated as needed treating all diversities. Actively demonstrates a passion for patient care while demonstrating service recovery if needed and displays service excellence at all times. Follows standard of care as defined within scope of Licensed Practical Nurse and reflects the American Nurses Association's Code of Ethics for Nurses and the Nurse Practice Act of Oklahoma. Maintains required licensures and certifications up to date as required by job description. Knows, understands and adheres to hospital/department policies and procedures. Follows established hospital/department safety policies. Attends mandatory in-services and pertinent seminars. Effectively communicates both orally and in writing in English. Demonstrates knowledge of hospital-wide approach to PI and how department is involved in program. Meets core competencies and department-based competencies (these core competencies maybe included in some departments competency program). Demonstrates use of basic documentation tools: Admission database Plan of care Patient/family education Vital signs Pain management Others as appropriate to the unit Able to identify changes in patient condition including nutritional monitoring, wound care, laboratory and diagnostic testing, etc., and reports significant findings to the RN. Adept in using escalation and the chain of command, Provides individualized care that supports the healing process. Implements orders for assigned patients that fall within the LPN scope of practice. Admissions Proactive receiving incoming patients: Greets all visitors with a smile and eye contact, acts as ambassador of facility. Identifies persons "coming to the unit" and initiates family education Rapid identification of 'sick' patient Assist with initiating documentation as appropriate Directs telephone inquiries to the nurse or specialty as necessary Performs central line dressing changes, including non-tunneled and tunneled catheters. Central line observations, including location and condition, are documented. Dressing changes are performed as per procedure. (A minimum of an initial competency is required) The professional nurse participates in patient progression along the continuum of care by collaborating with all disciplines. Communicates patient care needs to other disciplines (pertinent progress note documentation, involvement in care conferences, interdisciplinary rounds, and/or use of hospital protocols, coordination of nursing care activities, etc.). Acts as an advocate for patients who, by virtue of their disease, injury or life situation, may be vulnerable, compromised or disenfranchised. Utilizes nursing process to optimize patient outcomes (actively collaborates with RN in discharge planning, appropriate referrals, timely interventions, and care planning). Demonstrates a decision-making style (within the LPN scope of practice) that includes the input of the registered nurse. Assumes accountability and responsibility for self-direction. Integrates hospital policies and procedures into practice. Accepts responsibility for own actions and outcomes. Demonstrates knowledge of and holds self-accountable for their role in HCAHPs and the patient's experience. Completes hourly rounds to evaluate patient status, perception and quality of care provided, promote communication, and ensure safety in the environment. Document completion of hourly rounds. Promotes collaboration and partnership with physicians and assists with resolution of patient care questions and issues. Demonstrates competent clinical skills, critical thinking, and nursing judgment. Participates in annual skills fair. Accepts responsibility for the quality and level of care provided to all patients and evaluates appropriateness of care. Takes a patient assignment within their competency level. Constructs and implements plans of care using nursing knowledge and skill. Makes discretionary patients care decisions independently within the scope of practice. Is self-directed and decision oriented. Navigates the electronic medical record. Demonstrates competency in the use of technology and computers as necessary for the job. Participates in the design and implementation of systems that support effective teamwork. Serves as a positive role model for the team. Manages up team members and other departments. Communicates effectively with team to convey concerns, situations, or conditions relative to staff, patients and physicians and utilizes the chain of command. Provides complete end of shift report at the bedside including the patient and documents handoff. Provides plan of care daily. Participates in efforts to minimize cost and eliminate waste and unnecessary duplication. Demonstrates an awareness of the regulatory standards established by the Joint Commission, Federal, State and hospital standards. Demonstrates quality through accurate documentation of the nursing process, core measures to ensure compliance. Participates in review of patient charts and audits for core measure compliance, documentation requirements, guardrails, fall precautions, etc. Follows communication practices that minimize risks associated with handoffs and transitions in care, utilizing SBAR process. Performs patient identification following the hospital policy for any treatments or medications. Reports errors and near miss events to promote analysis and design of prevention or system improvements. Performs other duties as assigned. Qualifications Education: High School Graduate or equivalent License/Certification: Graduate of an accredited LPN program Completion of basic arrhythmia module within initial 90 days BLS (Current upon hire) Current LPN license in Oklahoma Venipuncture Skills required.
