Job Description:

ResponsibilitiesFunctions as patient and family advocate through initial and ongoing assessment of the patient's needs during hospitalization, identifying and coordinating community resources to meet continuing care needs.-- Thus facilitating the patient's movement through the hospital system, with assurance that their needs and concerns will be addressed.----

  • The Case Manager (C.M.) may meet with the patient and family prior to admission or at the time of admission, identifying continuing care issues, instructing the patient and assuring them that their concerns will be addressed.
  • The C.M. identifies groups of patients and barriers/delays within the system that contribute to the high cost of patient care.
  • The C.M. performs pre-admission/admission screening to determine the needs of patients, their appropriate level of care, 3rd party payer requirements and to initiate discharge planning assessment/education for quality continuum of care needs.
  • The C.M., through collaboration with the health care team, develops alternative patterns of care that will provide cost effective care without compromising quality.
  • The C.M. will be available during rounds to assist the physician with continuing care issues, providing information about available resources.
  • In collaboration with the health care team, the patient and family, the C.C. implements the discharge plan.
  • The C.M. will meet with the patient, family and physician on all active cases, as needed to assure continuity of care.
  • Reviews the medical record on all active C.M. cases, prior to physician rounds insuring that all orders are filled and results of test are available.
  • Serves as a patient advocate, acting as liaison between the patient, hospital and community resources.-- Maintaining patient autonomy in choice of providers.
  • Collects data to determine the cost and quality of necessary medical services.
  • Continuously assesses potential high-risk patients for complications and to insure services are in place at time of discharge for a safe transition.
  • The Case Manager will be available during rounds to assist the physician with continuing care issues, providing information about available resources.
  • Pre-Cert and update reviews on all procedures requiring intervention.--
  • Liaison with the Business Office regarding commercial insurance denials and appeals.
  • Quality/Risk Management Department's Case Manager's demonstrate competency annually by the on-going and continual care of the patient population they serve, including infant, pediatric, adolescent, adult, and geriatric.-- This includes assessment, reassessment and interventions for discharge planning and implementation of transfers.
  • Other duties as assigned.Qualifications
    • Current RN licensure in the State of Oklahoma required.
    • Two years' experience in discharge planning and utilization review desirable.--
    • Knowledge of QI helpful.
    • Must successfully complete the pre-employment assessment.
    • Excellent public relations skills required as well as communication with physicians, hospital staff, patients, families and community.--
